You can deduct medical mileage — miles driven to and from medical care — as part of your itemized medical-expense deduction, but only to the extent your total medical expenses exceed 7.5% of AGI (IRC §213). For 2025 the IRS standard mileage rates are 70¢/mile for business, 21¢/mile for medical and (Armed Forces) moving, and 14¢/mile for charitable driving (IRS Notice 2025-5).
